Appointment/Review Forms
The below packets are required for all new appointments and plan review requests. There are two options for completing these packets: a single printable PDF packet that you email/mail/fax back to us OR by using our secure online submission forms. The online submission forms are HIPPA compliant and sent securely directly to us. There is no printing required. They are the preferred method of submitting the information and very easy to use.
Please don't use the "Contact Us" contact form at the bottom of this page to request a plan review. The Contact Form is for new client inquiries, or if you have misplaced our email address and need to reach out via email.
• If you want a Part D Drug Plan review, we need either the printable "Part D Drug Plan Review (PDF)" filled out and email/mail/faxed back to us OR click on the link below to fill all forms out electronically and HIPPA compliantly.
• If you want a Part C Medicare Advantage Plan review, or would like to discuss MAPDs, we need either the printable "Part C Medicare Advantage Plan Review (PDF)" filled out and email/mail/faxed back to us OR click on the link below to fill all forms out electronically and HIPPA compliantly.
